Win/Loss Statement Request
Guests may obtain a Win/Loss statement by visiting Player Services or by submitting a Win/Loss Statement Request Form. The form must be signed and include the guest’s Player’s Club account number, date of birth, and a copy of a valid driver’s license or photo ID. Please mail or fax this request to the information on the form. Statements will be sent within 7–21 business days of receipt.
