Win/Loss Statement Request

Guests may obtain a Win/Loss statement by visiting Player Services or by submitting a Win/Loss Statement Request Form. The form must be signed and include the guest’s Player’s Club account number, date of birth, and a copy of a valid driver’s license or photo ID. Completed forms and photo ID should be emailed to antonious.rider@ballysbatonrouge.com. Statements will be sent within 7–21 business days of receipt. A Win/Loss Request Form will need to be added to the website.
